TO: THE DIRECTOR OF THE ARIZONA DEPARTMENT OF CORRECTIONS

Plaintiff Christian Ronnell Thomas, inmate #233382, who is confined in the

17

Arizona State Prison-Florence West, in Florence, Arizona, must pay the statutory filing

18

fee of $350.00. Plaintiff is not assessed an initial partial filing fee. Plaintiff must pay the

19

filing fee in monthly payments of 20% of the preceding month’s income credited to

20

Plaintiff’s trust account. The Director of the Arizona Department of Corrections or his

21

designee must collect and forward these payments to the Clerk of Court each time the

22

amount in the account exceeds $10.00.

IT IS ORDERED:

(1)

The Director of the Arizona Department of Corrections or his designee

25

must collect the $350.00 filing fee from Plaintiff’s trust account in monthly payments of

26

20% of the preceding month’s income the account. Payments must be forwarded to the

27

Clerk of Court each time the amount in the account exceeds $10.00. The payments must

28

be clearly identified by the name and number assigned to this action.

The Director of the Arizona Department of Corrections or his designee

must notify the Clerk of Court in writing when Plaintiff is released or transferred to a

3

correctional institution other than the Arizona State Prison Complex-Florence West, in

4

Florence, Arizona, so new billing arrangements may be made to collect any outstanding

5

balance.

The Clerk of Court must serve by mail a copy of this Order on the Director

7

of the Arizona Department of Corrections, 1601 West Jefferson, Phoenix, Arizona,

8

85007.

The Clerk of Court must forward a copy of this Order to Financial

10

Administration for the Phoenix Division of the United States District Court for the

11

District of Arizona. Financial Administration must set up an account to receive payments

12

on the filing fee for this action and must notify the Court when the filing fee is paid in

13

full.
