O’BRIEN, et al., 11 Defendants. / 12 13 Plaintiff Lynn Charles Beyett (“Plaintiff”) is a prisoner in the custody of the California 14 Department of Corrections and Rehabilitation (“CDCR”), proceeding pro se and in forma 15 pauperis in this civil rights action pursuant to 42 U.S.C. § 1983. This action is proceeding 16 against Defendants V. O’Brien and D. Ragan for deliberate indifference to a serious medical 17 need in violation of the Eighth Amendment. As of the date of this order, Plaintiff submitted the 18 form regarding consenting to or declining magistrate judge jurisdiction, choosing to consent. 19 Doc. 5. Defendants, however, have not submitted the form. 20 Accordingly, it is HEREBY ORDERED that 21 1. 22 23 declining magistrate judge jurisdiction; and 2. 24 25 26 Defendants are required to fill out and submit the form regarding consenting to or The Clerk of the Court is directed to send Defendants the Order Re Consent or Request For Reassignment.
