On October 27, 2008, Plaintiff filed an incomplete application to proceed in forma pauperis. Only page 1 of the fo r m was submitted. On November 5, 2008, the Court ordered Plaintiff to submit a completed application within 45 days. O v e r forty-five days have elapsed and Plaintiff still has not submitted a complete application or paid the filing fee. The Court s h a ll provide plaintiff with one final opportunity to submit his application to proceed in forma pauperis or pay the filing fee.

Plaintiff is a state prisoner proceeding pro se in a civil rights action pursuant to 42 U.S.C. § 1983. Plaintiff has not paid the $350.00 filing fee, or submitted a complete application to proceed in forma pauperis pursuant to 28 U.S.C. § 1915.1 Within twenty (20) days of the date of service of this order, plaintiff shall submit the attached application to proceed in forma pauperis, completed and signed, or in the alternative, pay the $350.00 filing fee for this action. No requests for extension will be granted without a showing of good cause. Failure to comply with this order will result in a recommendation that this action be dismissed. ///
