Petitioner's mother Jeanine Salcido, whose address is 220 Park Street, Madera, CA 93637, is a competent and responsible person, and fully competent to act as Petitioner's guardian ad litem.

Jeanine Salcido is willing to act as guardian ad litem for Petitioner, as appears by her consent attaced hereto.

WHEREFORE, Petitioner moves this Court for an order appointing Jeanine Salcido as guardian ad litem of Petitioner for the purpose of bringing suit against Defendants on the claim hereinabove stated.
