QUINONES, et al., (Doc. 62) Defendants. 15 16 17 Plaintiff, George E. Jacobs, IV, is a prisoner proceeding pro se in a civil rights action 18 pursuant to 42 U.S.C. ' 1983. On May 16, 2014, the Court issued an order authorizing the 19 issuance of a subpoena duces tecum. Fed. R. Civ. P. 4, 45; 28 U.S.C. § 1915(d). The requisite 20 notice having been issued pursuant to Federal Rule of Civil Procedure 45(b)(1), it is HEREBY 21 ORDERED that: 22 1. 23 Marshal (USM): 24 a. 25 26 The Clerk of the Court shall forward the following documents to the United States Two completed and issued subpoenas duces tecum to be served on: California Department of Corrections and Rehabilitation, Office of the Secretary 1515 S. St. Sacramento, CA 95814 27 28 1 Office of the Inspector General Custodian of Records/Executive Staff 10111 Old Placerville Rd. Sacramento, CA 95827 1 2 3 4 b. Two completed USM-285 form; and 5 c. Three copies of this order, one to accompany the subpoena and one for the 6 7 8 9 USM; 2. Within 20 days from the date of this order, the USM is DIRECTED to serve the subpoenas in accordance with the provisions of Rule 45 of the Federal Rules of Civil Procedure; 3. The USM SHALL effect personal service of the subpoenas duces tecum, along 10 with a copy of this order, upon the individual/entity named in the subpoena pursuant to Rule 45 of 11 the Federal Rules of Civil Procedure and 28 U.S.C. § 566(c); and 12 4. Within 10 days after personal service is effected, the USM SHALL file the return 13 of service, along with the costs subsequently incurred in effecting service, and said costs shall be 14 enumerated on the USM-285 form. 15 16 17 IT IS SO ORDERED.
