The petition of RACHELLE EDW A R D S as guardian ad litem of SEAN EDW A R D S to deposit f u n d s from the settlem e n t with defendant Lockhart in a blocked account or blocked accounts cam e on for h e a r i n g on March 23, 2009 at 9 a.m . THE COURT ORDERS: 2 . Money that belongs to SEAN EDW A R D S shall be deposited in an interest-bearing, federally in s u r e d blocked account or accounts. 3 . Each account shall indicate the nam e of the m in o r or other person who owns the account. 4 . The total am o u n t authorized for deposit, including any accrued interest, is: $5,364.37 to a blocked FDIC insured bank account in the nam e of "Rachelle Edwards, as Guardian ad lit e m of Sean Edwards." 5 . W ith d r a w a ls : The blocked account or accounts belong to a m in o r . The m in o r was born on J u ly 8, 1995. No withdrawals of principal or interest shall be m a d e from the blocked account or accounts w ith o u t a written order under this case nam e and num b e r , signed by a judge, and bearing the seal of this c o u r t, until the m in o r attains the age of 18 years. W h e n the m in o r attains the age of 18 years, the d e p o s ito r y, without further order of this Court, is authorized and directed to pay by check or draft directly to the form e r m in o r , upon proper dem a n d , all m o n e ys including interest deposited under this order. The m o n e y on deposit is not subject to escheat. 6 . The petitioner and the petitioner's attorney, if any, shall deliver a copy of this order to each d e p o s ito r y in which funds are deposited under this order. The depository's acknowledgm e n t of receipt of t h e order and the funds shall be filed with this court within 15 days of deposit.
