Plaintiff, a prisoner without counsel, filed a complaint alleging civil rights violations.

pauperis pursuant to 28 U.S.C. § 1915(a)(1).

found that plaintiff’s application to proceed in forma pauperis failed to include the certified copy

of his trust account statement or institutional equivalent required by 28 U.S.C. § 1915(a)(2). The

court ordered plaintiff to submit the required trust statement within 30 days and warned him that
23
failure to do so would result in a recommendation that this action be dismissed.

Accordingly, it is hereby RECOMMENDED that this action be dismissed without
prejudice.

assigned to the case, pursuant to the provisions of 28 U.S.C. § 636(b)(l). Within fourteen days
