On October 6, 2009, plaintiff filed this pro se civil rights action along with an application to proceed in forma pauperis. The same day, the clerk of the Court sent a notification to plaintiff that the application was insufficient because plaintiff did not use the correct form. (Docket No. 3.) On October 19, 2009, plaintiff filed a motion for extension of time to file the in forma pauperis application. (Docket No. 5.) Good cause appearing, plaintiff's motion is GRANTED. Plaintiff must submit a completed In Forma Pauperis Application using this court's current form within thirty (30) days from the date this order is filed. Failure to do respond in accordance with this order will result in dismissal of this case without prejudice for failure to pay the filing fee without further notice to plaintiff.
