The amended proposed class notice is APPROVED subject to the following revisions.

First, the parties have indicated that they are “still waiting for confirmation of contact

19

information for the class administrator.” In the proposed notice, however, they have provided

20

contact information for a class administrator. If this is not, in fact, the correct contact

21

information, class counsel shall input the correct information in the final notice prior to mailing.

22

Second, the parties shall delete the words “or currently live” from the final paragraph of page 4

23

of the notice. Third, on page 6 the parties shall rewrite the second directive under number 9 so

24

that it states as follows: “2) Be sure that all adult household members seeking to object sign the

25

letter and state their relationship to any child in the household who is also a member of the

26

class.”

If the parties make any further modifications, they shall file a redlined copy reflecting
those changes by JULY 18. The notice shall be mailed by no later than JULY 24.
Upon mailing the notice, the parties shall file a copy of the notice that was mailed along

with a declaration certifying how it was mailed and including the names and addresses of all

5

individuals it was mailed to. At the parties’ request made by proper motion, names and

6

addresses will be filed under seal.
