Plaintiff has not yet done so. Therefore, IT IS HEREBY ORDERED that no later than June 23, 2009, Plaintiff shall file either a "Consent to Proceed Before a United States Magistrate Judge," or else a "Declination to Proceed Before a United States Magistrate Judge and Request for Reassignment." IT IS FURTHER ORDERED that the hearing on Defendant's motion to dismiss is rescheduled for July 7, 2009 at 10:00 a.m. in Courtroom 5 of this court. IT IS FURTHER ORDERED that the deadline for Plaintiff to serve and file a written opposition to Defendant's motion is extended to June 23, 2009.
