M i s s e t t , M . D . a n d William J. Lynch, Ph.D., on April 16, 2010, or on another date to be mutually agreed upon by the parties. The examination is to begin at 9:00 a.m and is scheduled to last a full day. It will take place at Dr. M issett's office, located at 1187 University Drive, M enlo Park, California 94025; telephone (650) 326-5564. Dr. M issett is Board Certified in psychiatry, addiction psychiatry, and forensic psychiatry by the American B oard of Psychiatry and Neurology and Dr. Lynch is Board Certified in Clinical Neuropsychology. Dr. M issett will perform a psychiatric evaluation of the plaintiff which will include a mental status examination. Dr. Lynch will perform neuropsychological testing and will s e r v e a s a p s y c h o l o g i c a l c o n su l t a n t t o D r . I t w i l l i n v o l v e n o i n v a s i v e , d a n g e r o u s o r p a i n fu l p h y s i c a l p r o c e d u r e s . The first part of the examination will be personally conducted by Dr. Missett in his office. It will consist of the following: the taking of the individual's personal and family history; a medical a n d p s y c h i a t r i c h i s t o r y; a n e d u c a t i o n a l a n d w o r k h i s t o r y ; a s o c i a l , m a r i t a l , r e l a t i o n a l , a n d r e c r e a t i o n a l h i s t o r y , a m e d i c a t i o n a n d su b s ta n c e - u s e h i s t o r y ; a l e g a l h i st o r y ; a h i s t o r y o f t h e incid e n t( s) r esu ltin g in th e claim s o f p sych o lo g ical an d / o r e m o tio n a l in ju r ies a n d /o r d istr ess; a history of the changes over time in the claimant's psychological and/or emotional signs or symptoms s i n c e t h e o n s e t o f a n y p s y c h o l o g i c a l o r e m o t io n a l d i s t r e s s r e p o r t e d ly c o n n e c t e d w i t h t h e i n c i d e n t ( s ) ; the extent to which any psychological and/or emotional signs or symptoms interfere with the claimant's daily functioning; the history of any such interference over time; the psychological and/or em o tio n a l c o n d itio n o f the claim a n t as o f th e d ate o f th e ev alu a tio n ; the relatio n sh ip , if an y, b e tw een t h e n a tu r e a n d i n t e n s i t y o f a n y p s y c h o l o g i c a l a n d / o r e m o t i o n a l s i g n s o r s y m p t o m s r e l a t e d to t h e c l a i m e d i n c i d e n t ( s ) ; a n y p r i o r , c o n t e m p o r a n e o u s , o r s u b s e q u e n t e x p e r ie n c e o f a n y p s y c h o l o g i c a l and/or emotional predisposing and/or distressing events in the individual's life; the claimant's usual d a il y s c h e d u l e a s o f th e t i m e o f t h e e v a l u a t i o n ; w h e t h e r t h e c l a i m a n t e v i d e n c e s t h e n - c u r r e n t l y a n e e d for psychiatric and/or psychological counseling or psychotropic m edication for the alleviation or relief of any psychological or emotional signs or symptoms or any interference in daily functioning that might be connected with the incident(s); and the claimant's prognosis for the future. The first part of the exam in ation will also include the review of the claimant of any written documentation connected with the claimant's report of her psychological and/or emotional injuries or damages and the possible causes thereof. Th is first p art of the examination is expected to last approximately three hours from 9:00 a.m. to noon. T h e se i n c l u d e p o r t i o n s o f t h e W e c h s l e r A d u l t I n t e l l ig e n c e S c a l e ­ 3 r d e d i ti o n ( W A I S I I I ) , t h e W e c h s l e r A b b r e v ia te d S c a l e o f I n te ll i g e n c e ( W A S I ) , t h e S h i p l e y I n s t i t u t e o f L i v i n g S c a l e (SILS), the Kaufman Brief Intelligence Test (KEIT), and the M icroCog Assessment of Cognitive F u n c tio n in g ( M -C o g ) . In the assessment of the plaintiff's neurocognitive status, portions of as many as four or five of the following, most commonly used neurocognitive measu res may be administered. These include: the Halstead-Reitan Neuropsychological Test Battery (HRNTB); the Trail Making Test P a r t s A a n d B (T M T ) ; th e S tr o o p N e u r o p syc h o lo g i c a l S c r e e n i n g T e s t ( S N S T ) ; t h e R e p e a t a b l e Battery for the Assessment of Neuropsychological Status (RB A N S ); the Neuropsychological Assessment Battery (NAB) that consists of a Screening M odule as well as five main modules having to do with measures of attention, language, memory, spatial functions, and executive functions; the W e c h s le r M e m o r y S c a l e - 3 r d E d i t i o n ( W M S - I I I ) ; t h e M e m o r y A s se ss m e n t S c a l e s ( M A S ) ; t h e Wisconsin Card Sorting Test (WCST); and the California Verbal Learning Test - II (CVLT-II). With respect to an assessment of emotional functions and the plain tiff's personality, her p e r s o n a l i t y st a t u s a n d a t t it u d e t o w a r d s i l ln e ss w i l l b e e x a m i n e d t h r o u g h t h e a d m i n i s t r a t i o n o f o n e or two of the following measures: the M innesota M ultiphasic Personality Inventory-II (M M PI-II), t h e M i l l o n C l i n i c a l M u l t i a x i a l I n v e n to r y - I I I ( M C M I - I I I ) , t h e M il l o n B e h a v i o r a l M e d i c i n e Diagnostic (M BMD), the Battery for Health Improvement - II CBHI-II), the Personality Assessment I n v e n t o r y ( P A I ) , a n d t h e S y m p t o m C h e c k l i s t 9 0 - R e v is e d ( S C L - 9 0 - R ) . The persons allowed to be present for this exam ination w ill be limited to plaintiff and the examiners. Plaintiff will be allowed to tape record any interview portion of the examinations.
