On May 2, 2011, Plaintiffs Sterling Bindel by and through her Guardian Ad Litem Kirsten

Tarman, Monique Bindel by and through her Guardian Ad Litem Kirsten Tarman and Kirsten
20
Tarman, individually, moved to remand. This case has been assigned to the undersigned magistrate
21
judge. Before the court takes any further action in the case, the court must determine whether or not
22
all of the parties who have appeared consent to magistrate judge jurisdiction by the undersigned, or
23
whether any of the parties request reassignment to a district judge. Therefore,
24
IT IS HEREBY ORDERED that no later than May 13, 2011, each party shall file either a
25
“Consent to Proceed Before a United States Magistrate Judge,” or else a “Declination to Proceed
26
Before a United States Magistrate Judge and Request for Reassignment.” Both forms are available
27
from the Clerk of the Court, or from the Forms (Civil) section of the court’s website at
