In order to proceed with this case, Plaintiff must either pay the full filing fee of $350.00 or file a properly completed in forma pauperis application. If he chooses to submit a new in forma pauperis application, he should indicate the sources of any money he has received within the past twelve months and state the amount received from each source during the past twelve months. Additionally, he shall estimate the total value of cash or funds in all bank accounts that he owns so that the court has sufficient information upon which to consider his eligibility to proceed in forma pauperis.
