On 6/2/11 I began the process of submitting a PHV application. I submitted a credit car payment,
which was accepted. However, when I reached the final screen I realized I had listed the wrong party
for who the applicant would be representing. At that point I stopped the process and I did not submit
the PHV application, thus it does not appear on the docket. I instead started over, and selected the
correct party, and paid again.
Thus I ask that this charge be refunded since I paid for the PHV fee but never actually submitted it.
