APPLICATION FOR REFUND OF FEES PAID ELECTRONICALLY THROUGH PAY.GOV Date of Request: 6/3/11 Date of Fee Payment: 6/2/11 Case Number: Receipt Number: 253H3A34 or 113E-3334183 1:11-cv-01804-TWT Amount to be Refunded: 150 Reason for Request: On 6/2/11 I began the process of submitting a PHV application. I submitted a credit car payment, which was accepted. However, when I reached the final screen I realized I had listed the wrong party for who the applicant would be representing. At that point I stopped the process and I did not submit the PHV application, thus it does not appear on the docket. I instead started over, and selected the correct party, and paid again. Thus I ask that this charge be refunded since I paid for the PHV fee but never actually submitted it.
