MEMORANDUM AND ORDER IT IS HEREBY ORDERED that plaintiff's motion to proceed in forma pauperis [Doc. # 2 ] is GRANTED. IT IS FURTHER ORDERED that plaintiff shall have until October 11, 2013, to file an amended complaint on a court-provided form in compliance with the instructions set forth above. IT IS FURTHER ORDERED that if no amended complaint is filed by the above deadline, this action will be dismissed without further notice to plaintiff. IT IS FURTHER ORDERED that the Clerk of Court shall send a civil complaint form to plaintiff along with a copy of this Order.
