Plaintiff Todd Tolbert (“Plaintiff”) and Defendants Cox Enterprises, Inc. (“Cox

Enterprises”) and Aetna Life Insurance Company (“Aetna”) (Cox and Aetna are collectively

referred to herein as “Defendants”) (Plaintiff and Defendants are collectively referred to herein
