OPINION AND ORDER re: 18 MOTION to Dismiss filed by Raul Ulloa, Westchester County, Alan Peschiera, Correct Care Solution LLC, Joon Park. The motion to dismiss is GRANTED. Plaintiff shall file an amended complaint, utilizing the form attached hereto, by January 7, 2019. The Clerk is directed to (i) terminate the pending motion (Doc. #18), and (ii) mail a copy of this Order and the amended complaint form attached hereto to plaintiff at the address on the docket. The Court certifies pursuant to 28 U.S.C. § 1915(a)(3) that any appeal from this order would not be taken in good faith, and therefore in forma pauperis status is denied for the purpose of an appeal.
