DECISION AND ORDER re 2 MOTION for Leave to Proceed in forma pauperis filed by Anthony L. Breedlove. In order for the Court to grant the request, it needs sufficient information on which to base its determination. The petitioner must either submit a supplemental application form with a completed and signed prison certification or pay the $5.00 filing fee. The Clerk of the Court is directed to send the petitioner an application to proceed as a poor person. The petitioner is directed to su bmit the application or pay the filing fee by 7/23/09. If the petitioner fails to comply with this Order by 7/23/09, the Clerk is directed to close the case as dismissed without prejudice without further order of the Court. Signed by Hon. Charles J.
