ORDER adopting the Magistrate Judge's 10/15/12 Initial Screen Report and Recommendation. Plaintiff's claims against Warden Bradshaw, Major Mapp, and defendant Sgt. Berkahart remain pending. Plaintiff's claims against defendants DesMa rais, Floyd, Granson, Mayo, Rose and Bernard failed to give defendants adequate notice of his claims, and these defendants are dismissed. The Clerk is directed to terminate defendants Parks, Jordan, Betha, White, Au, Coey, Thomas, Burgess, Phillips, Putt and Fannon.
