MEMORANDUM DECISION reversing the decision of the Commissioner of the Social Security Administration and remanding case to the Commissioner of the Social Security Administration. On remand, the Commissioner should re-evaluate the med ical evidence in light of the correct number of migraines and injections Plaintiff experienced during the relevant time period, and the corresponding effects such migraines and injections had on Plaintiff.
