Replacement copies for lost books may be accepted in lieu of paying the fee provided the replacement is the same edition and in good physical condition. If the original is ultimately located, the patron will not be reimbursed for the cost of the replacement nor can the replacement copy be returned to the patro~. Lost books remain the property of the University of Wisconsin-Madison and must be returned ifsubsequently located. Please present photo ID with arrrent address information No borrowing card issued when Card Application Desk is closed. ~ / ~ ~ /-~ University of Wisconsin-Madison Memorial Library COMMUNI1Y BORROWERS CARD (ANNUAL FEE) Adult residents ofWisconsin are eligible to borrow books from the OW-Madison General Library System. Two forms of identification, plus a $30.00 annual fee ($15.00 for citizens over age 62) must be presented to the Card Application Desk. One form of ID must have a recognizable photo, both forms must have current address information, showing Wisconsin residency. Checks, credit or debit cards will be accepted at the Card Application Desk for payment of the annual fee. The borrowing card is issued on a fiscal year basis and expires each JUne 30. Cards may be renewed annually by again presenting ID showing current WISCOnsin residency, provided that there are no outstanding replacement or processing fees. All such charges must be paid before the card is renewed. CIRCUlATION POUCIES A currently valid Memorial Library card must be presented at the time of checking out or renewing library materials. The borrower is responsible for all books checked out on his or her borrowing card. The borrowing card is not transferable. Entering an ID number and the last name will bring up a list of the items currently charged out to the patron. By clicking on the white box next to each item, the material may be renewed. In some cases, the item may not be renewed. Therefore, it is important to make sure that the screen says, "renewed" in the far right-hand column. 2. Timberlake (Case No. 08-cv-0670) (now called "West v. Smith"). West v. Smith is a class action in the federal court for the Eastern District of Wisconsin. The federal court for the Eastern District of Wisconsin has directed that this notice be sent. This notice tells you about this case. Please read it carefully. What is a class action? A class action is a lawsuit filed by a person or persons on behalf of other people who have the same complaints. The persons who filed West v. Timberlake are residents of Milwaukee County who were having problems getting Medical Assistance, BadgerCare or FoodShare. (FoodShare is also called Food Stamps.) The persons who filed this case are representatives of all people in Milwaukee County who have the same complaints. Who is in the class? The class is defined as "all eligible applicants for, and recipients of, FoodShare, Medical Assistance (MA), and BadgerCare Plus who reside in Milwaukee County." The class does not include some applicants and recipients. The persons who are not included are: "MA recipients who also receive Supplemental Security Income (SSI) or foster care; and, applicants for, and recipients of, Katie Beckett MA, SeniorCare, BadgerCare Plus Core Plan for Childless Adults, Family Care, or a home and community-based waivers program under section 1915 of the Social Security Act." What problems did the class representatives complain about? The lawsuit was filed because applications for MA, BadgerCare or FoodShare were not processed or open cases were closed for no review; verification documents were not processed; six-month report forms (Six Month Reviews) were not processed; and class members could not reach workers in the Milwaukee County Department of Human Services (Milwaukee County) to get problems with their cases corrected. The lawsuit asked the Court to order Milwaukee County and the State of Wisconsin Department of Health Services (the State) to follow federal Medical Assistance and Food Stamp laws that require applications, reviews and Six Month Reviews to be processed within certain time limits. What happened after the lawsuit was filed? The State and the attorney for the class signed an agreement in April, 2009. In that agreement, the State agreed to take a number of steps to improve application, review and verification processing in Milwaukee County. After the agreement was signed: The State established a telephone call center staffed with over 60 people where questions about cases are answered. The State reports that more than 90% of applications are decided within 30 days. The State reports that more than 90% of reviews are decided in the review month or within 10 days. The State reports that more than 90% of FoodShare Six Month Reviews are processed within 10 days. The State reports that more than 90% of verification documents are processed within 10 days. The State prepared new notices telling people about missing verification and explaining what verification is needed. The defendants are not saying that they violated any requirements of federal or state law. Why are you receiving this notice now? The State and the attorney for the persons who filed the lawsuit are now asking the federal court to dismiss this case. If the court dismisses the case, the case will be over. The attorney for the persons who filed the lawsuit will also sign a document releasing the defendants from any liability. This means that class members cannot sue the defendants for these same problems. As a member of the class, you can object to the plan to dismiss the lawsuit if you do not believe that it should be dismissed. You cannot ask to "opt-out" (not be included in the class). How does a class member object? As a member of the class, you may file an objection. You need to explain your reasons for objecting in writing.
