Filing

ORDER on ifp request: Financial Affidavit due 4/29/2016. Signed by Magistrate Judge Peter A. Oppeneer on 4/8/2016. (jef),(ps)
IN THE UNITED STATES DISTRICT COURT
FOR THE WESTERN DISTRICT OF WISCONSIN
MARYAM E. MUHAMMAD ,
Plaintiff,
ORDER
v.
Case No. 15-cv-41-wmc
BEVERLY LOUIS, et al.
Defendants.
On January 21, 2015, this case was closed because plaintiff had not paid a $350
sanction in case no. 13-cv-831. Plaintiff has now paid the sanction and has filed a motion to
reopen this case. Plaintiff also asks for leave to proceed without prepayment of fees or costs.
However, I cannot determine whether plaintiff qualifies for indigent status. To make this
determination, plaintiff must submit a financial affidavit, which I am enclosing with this
order.
ORDER
IT IS ORDERED that plaintiff Maryam E. Muhammad, may have until April 29,
2016, in which to submit an affidavit of indigency and return it to the court. If plaintiff fails
to provide this requested information in a timely fashion, then the court will deny plaintiff’s
request for leave to proceed without prepayment of fees or costs for failure to show that
plaintiff is indigent.
Entered this 8th day of April, 2016.
