SELECTION OF THE JURY IN A CIVIL CASE
The parties to this action hereby waive their right to proceed before a United States district judge
and consent to have a United States magistrate judge conduct voir dire and supervise the selection of the
jury in this action. The opportunity to seek de novo review of any decision of the Magistrate Judge made
during jury selection to a district judge is also expressly waived. An appeal of jury selection decisions
will lie to the court of appeals as it would if made by a district judge.

ORDER OF REFERENCE
It is hereby ORDERED that this action be referred to U.S. Magistrate Judge Cassady for the
selection of the jury pursuant to 28 U.S.C. ยง 636(b)(3) and Local Rule 72.1. See Peretz v. United States,
501 U.S. 923, 111 S.Ct. 2661, 115 L.Ed.2d 808 (1991). Any appeal of a decision of the magistrate judge
made during jury selection will lie to the court of appeals as it would if made by a district judge.
