Guillermo Cruz Trujillo (APlaintiff@) is a state prisoner proceeding pro se and in forma

pauperis in this civil rights action pursuant to 42 U.S.C. ' 1983.
proceeding on Plaintiff’s Third Amended Complaint, which was filed on February 16, 2016.
(ECF No. 17).
On February 6, 2017, Plaintiff filed a motion requesting a USM-285 Form and a copy

of the Third Amended Complaint. (ECF No. 53). Plaintiff states that these documents were
not attached when he received the Court’s order to complete and return the service documents.
Because Plaintiff needs these documents in order to complete and return the documents

This case is currently
necessary for service of process, the Court will grant Plaintiff’s motion. The Court notes that
when Plaintiff completes the summons and USM-285 form, he is to omit the address for
defendant Juarez. (ECF No. 38, p. 2).
