Gonzalez, a state prisoner proceeding pro se, filed this action pursuant to 18 42 U.S.C. § 1983 on December 31, 2014. Plaintiff filed an application to proceed in forma 19 pauperis pursuant to 28 U.S.C. § 1915, but the form he used does not include the requisite 20 language acknowledging the fee will be deducted in increments from his trust account if funds are 21 available. Therefore, Plaintiff shall sign and file the application form provided with this order. 22 Plaintiff does not need to have the trust account officer complete the certification or submit a trust 23 account statement. 24 Accordingly, IT IS HEREBY ORDERED that: 25 1. pauperis by a prisoner used by this court; 26 27 28 The Clerk’s Office shall send Plaintiff the form for application to proceed in forma 2. Within forty-five (45) days from the date of service of this order, Plaintiff shall sign and file the application to proceed in forma pauperis; and 1 3. 2 Failure to comply with this order may result in a recommendation to the district court that this action be dismissed. 3 4 5 IT IS SO ORDERED.
