JAMES CARLSON, Defendant. 15 / 16 17 18 On December 14, 2011, Plaintiff, a state prisoner 19 incarcerated at San Quentin State Prison in San Quentin, California, 20 filed this pro se civil rights complaint under 42 U.S.C. § 1983. 21 That day, the Clerk notified Plaintiff that his in forma pauperis 22 application was incomplete because he failed to submit a Certificate 23 of Funds in Prisoner’s Account completed and signed by an authorized 24 officer at the prison. 25 On January 25, 2012, Plaintiff submitted monthly Inmate 26 Statement Reports from 2011, and informed the Court that he has 27 attempted to obtain a completed and signed Certificate of Funds in 28 Prisoner’s Account without success.
