JUROR QUESTIONNAIRE
You have been called to serve as a juror in a civil case entitled Realtek Semiconductor
versus LSI Corporation and Agere Systems LCC. The right to a jury trial is an important
protection enjoyed in the United States. The preservation of this right depends upon the
cooperation of qualified jurors in serving on juries and acting as judges of the facts. Serving on a
jury is always something of a burden but one that should be educational, interesting and
satisfying and which as good citizens we must occasionally bear.
The information you provide on this form and later in response to additional oral
questions asked in the courtroom will assist the court in determining whether or not you should
be excused from service on the jury for some legal cause and will allow the parties to learn some
information about you so they can exercise their right to make selections. Please do not discuss
any of the questions or your responses with anyone except as the court may permit during the incourt selection process.
