TAKE NOTE:
Notice is hereby given that Paul Burritt, plaintiff in the above named case, per Fed.
R. App. 3(a) and 7th Cir. R. 3, hereby appeals to the United States Court of Appeals for the
Seventh Circuit from the order denying relief per Fed. R. Civ. P. 59(e) from summary
judgment in the favor of Defendants on 25 February 2014, where said order denying relief
per Fed. R. Civ. P. 59(e) was entered in this action on the 30th day of March, 2015.

• 7th Cir. R. 28(a)(1) district court statement: The district court has subject matter
jurisdiction per 28 U.S.C. §§ 1331 (federal question) and 1367 (supplemental
jurisdiction over claims under Wisconsin law)
• 7th Cir. R. 28(a)(2) appellate court jurisdictional statement: (a) The United States
Court of Appeals for the Seventh Circuit has jurisdiction per Fed. R. App. P. 3 and
4 as an appeal of an order of the U.S. District Court for the Western District of
Wisconsin dated 30 March 2015 [Docket 77] denying Plaintiff’s timely motion per
Fed. R. Civ. P. 59(e) [Docket 69] to alter or amend, or to seek relief, from the
judgment entered by the district court on 25 February 2014 granting summary
judgment in the favor of the defendants against the plaintiff on all federal law claims
of the plaintiff [Docket 66]. (b) The plaintiff filed his notice of appeal on 26 April
2015 from the 30 March 2015 order. (c) No claims remain for disposition by the
district court.
